SPORTS PHYSICALS will be held at Grandville Middle School for the 2023-24 sports season.
These are offered for all incoming 7th-12th graders. 
 
Date:  May 10 and May 17, 2023

Time:  5:30 - 7:30 pm (no appt. needed)

Location: Grandville Middle School. Enter the main doors by the flagpole. 

Cost:  $25.  Cash or check (written to Dr. Diana Dillman). NO CREDIT CARDS ACCEPTED

Form needed: An MHSAA approved physical form can be preprinted from FinalForms at https://grandville-mi.finalforms.com/ Once signed in, CLICK TO PRINT - MHSAA PHYSICAL FORM, and all the info you entered will print for the doctor plus a blank form. Bring this paperwork to your physical.

(Note: If preferred, you may have your family physician do the physical. Take the correct form to your physician. ALL physicals must be dated April 15, 2023, or after to be valid for the 2023-24 sports season.)
