SPORTS PHYSICALS will be held at the Middle School for the 2022-23 sports season.
These are offered for all incoming 7th-12th graders.
 
**NOTE:  If parents prefer, you can get the physical done by your own family physician.**
 
Date:  May 18 and May 24, 2022
Time:  5:30 - 7:30 pm.  (no appt. needed)
Cost:  $25.  Cash or check (written to Dr. Diana Dillman)  NO CREDIT CARDS ACCEPTED
Form needed:  Physical Form  Please bring completed form to the physical.  
(Forms can also be picked up prior to the physical in the main office)
 
Location:  Please enter the doors at the north end of the building (the sports entrance by the pool/auditorium).  Turn left when you enter the building and cut through the gym (on the right)
to arrive at the band wing.  Physicals will be held in the band practice rooms.  Someone will
be in the band wing to direct you. 
 
IMPORTANT NOTE TO PARENTS - ALL physicals must be dated April 15, 2022, to be
valid for the 2022-23 sports season.  So even if your child had a physical on April 14, they
will need a new one per MHSAA rules. Contact Kate Breazeale if you have any
